MEDICAL AND RURAL HEALTH SERVICES

??????????? The Director of Medical and Rural Health Services is in charge of planning and execution of all programmes of Medical Services.? He is responsible for rendering medical care services, through the grid of 25 District Headquarters Hospitals, 161 Taluk Hospitals, 79 Non-taluk Hospitals, 12 Dispensaries, 11 Mobile Medical Units, 7 Women and Children Hospitals, 5 TB Hospitals/Sanatoria, 2 TB Clinics and 1 Leprosy Rehabilitation Promotion Unit.?

?The Joint Directors of Health Services in all Districts except Chennai are responsible for the supervision and the implementation of all the Medical Health Programmes including Family Welfare.? They also coordinate with the other departments to implement the programmes such as Control of Blindness, Prevention of AIDS, Hill Area Development Programme, and Health Schemes for Adidravida and Tribal Welfare.

??????????? The Hospitals under the control of the Director of Medical and Rural Health Services are having a total bed strength of? 20,601.? The daily average inpatient strength is 15,998 and average out patient strength is 1,64,017.

??????????? Improvements to district, taluk and non-taluk hospitals play an important role in ensuring access of health services to the poor.?

??????????? The proposed Health Systems Development Project posed to the World Bank for funding at a cost of Rs.650 crores focuses on improvements to district and sub-district hospitals.? The project consists of three main components.

To render immediate medical care to road accident victims and to safeguard their life, the following Accident and Trauma Care Centres are being strengthened at a cost of Rs.338 lakhs.

Besides this, the Accident and Trauma Care Centre at the Headquarters Hospital, Perambalur, which is situated on the National Highway and is prone to frequent major accidents is being strengthened at a cost of Rs. 105 lakhs.? ????????????????

In order to provide advanced Medical Diagnostic Services to the General Public, Regional Diagnostic Centres are being established in the following 7 District Headquarters Hospitals, with latest Diagnostic equipments, at a cost of Rs.21 crores.

TB Control Programme
?
There are about 10 lakhs TB patients in Tamil Nadu.? There are 10 TB Sanatoria and TB Hospitals in the State.
The Revised National Tuberculosis Control Programme (RNTCP)is now implemented in all Districts of the State. The Directly Observed Treatment Short course (DOTS), recommended by World Health Organisation (WHO) for detection and cure of TB, has replaced the conventional institutional treatment.? Under this strategy tuberculosis patients are given short duration treatment with powerful anti TB drugs under the supervision of trained personnel.? Due to this cost effective intervention, the State has achieved a sputum conversion rate of 91% and a cure rate of 84%, in line with expected performance parameters under RNTCP.? With the active involvement of all medical institutions, Non-Governmental Organisations, private practitioners and the community in RNTCP, the incidence of Tuberculosis in the State can be brought down.

Pre-Natal Diagnostic Techniques (Regulation and Prevention of Misuse) Act 1994, came into force in Tamil Nadu with effect from 01.01.1996.? For effective implementation of this Act , State, District and Taluk level Advisory Committees have been formed.? The Director of Medical and Rural Health Services, the Joint Directors of Health Services and Senior Civil Surgeons / Medical Officers of the Government Taluk Hospitals have been nominated as respective Appropriate Authorities to implement the Act.? The total number of Scan Centres registered so far is 2034.? Due to constant vigil, 79 unregistered Scan Centres have been seized and sealed.? Cases have been filed against 21 Scan Centres and Judgments delivered for 16 cases convicting the offenders with fine.

District Mental Health Programme
?
Mental ill-health is an issue of Public Health importance in the community.? It has to be approached in a? sensitive manner.? Hence the District Mental Health Programme is being implemented in select districts.? The scheme outlay is? Rs.1 crore for a Programme period of five years.? The objectives of the programme are early identification of mental illness in the community and mental health care.? Training of medical, para-medical and non-medical personnel, mental health care delivery and awareness programmes are different components of the programme.? The Director of Medical and Rural Health Services is the controlling authority and the Institute of Mental Health is the Nodal Agency for implementation of the Programme.? At present the District Mental Health Programme is being implemented in Tiruchirappalli, Madurai and Ramanathapuram Districts.? This Government has approached the Government of India to sanction Rs.10 crores as Central Assistance to implement the District Mental Health Programme in the 9 districts of Tirunelveli, Theni, Thanjavur, Perambalur, Tiruvallur, Kancheepuram, Vellore, Salem and Tiruvannamalai.

Tamil Nadu State Illness Assistance Society

The Government of Tamil Nadu has constituted the Tamil Nadu State Illness Assistance Society with an initial corpus fund of Rs.15 crores. The funding pattern will be in the ratio of 2:1 between State Government and Government of India. ?The affairs of the Tamil Nadu State Illness Assistance Society is administered by the Executive Committee under the? Chairmanship of the Secretary to Government, Health and Family Welfare Department and the Director of Medical and Rural Health Services as the Member Secretary.? The Society provides financial assistance for surgical treatment in notified Government Hospitals and provides financial assistance through the 29 District Collectors to the poor patients for undergoing life saving surgeries through the private accredited hospitals recognized by Government on the lines of the Chief Minister? Public Relief Fund scheme.
