MEDICAL AND RURAL HEALTH SERVICES

?????????????????????????????????????????????????????????????????????????????????????????????????????????The Director of Medical and Rural Health Services is in-charge of planning and execution of all programmes of Medical Services in the Districts. He is responsible for rendering Medical Care Services, through the grid of 29 District Headquarters Hospitals, 156 Taluk Headquarters Hospital, 80 Non-Taluk Hospitals, 12 Dispensaries, 11 Mobile Medical Units, 7 Women and Children Hospitals, 2 T.B Hospitals, 2?? T.B. Clinics and Leprosy Hospitals. The Director of Medical and Rural Health Services is the executive authority on the policy decision taken by the Government from time to time.? He also provides the health delivery system mainly curative aspects through the Medical institutions under his control. He is also looking after the implementation of various schemes like Human organ Transplantation Act and Private Clinical Establishment Regulation Act, Pre-Natal Diagnostic Techniques Act, Recognition of private hospitals etc.,

The State has been divided into 30 Revenue Districts and in 29 Districts, Medical Care Services are implemented through this Department excepting Chennai District. For administrative convenience the medical institutions in? this 29 District have been divided in to 42 Health Unit Districts and the Medical Institutions in each Health Unit District is supervised either by a Joint Director of Medical and Rural Health Services and Family Welfare or by a Deputy Director of Medical and Rural Health Services and Family Welfare.

The Deputy Director of Medical Services (Thoracic Medicine) in every District looks after the T.B. Control Programme. The Deputy Directors of Medical Services (Leprosy) in each District look after the Leprosy Eradication Programme. The Leprosy Programme has already been integrated with the Department of Public Health and Preventive Medicine to involve Leprosy field staff in Public Health work and the Health Inspectors in Leprosy work.

Improvements to District, Taluk, Non-Taluk Hospitals and Dispensaries constitute important development activities in the shape of expanded Medical facilities, specialised services such as Medicine, Surgery, Obstetrics and Gynaecology, Ophthalmology, ENT, Venerology, Orthopaedic Surgery, Anesthesiology, Child Health, Dental, Psychiatry, Pathological Laboratory Services, Leprosy, T.B., Diabetic, Cardiology, Nephrology Specialists in the rank of Senior Civil Surgeons are available in the District Headquarters Hospitals. Accident and Emergency Services are available to meet the accident cases. The Maternity and Child Health and Family Welfare activities are undertaken effectively through non-teaching Medical Institutions.

The other Medical Health Programmes, such as control of Blindness, Medical facilities under Hill Area Development Programme, Adi Dravidar and Tribal Welfare Programme etc., are being coordinated and implemented by the Department of Medical and Rural Health Services through the Government institutions functioning under the control of this Department.

Based on the policy decision taken by Hon'ble Chief Minister 25 Operation Theatres in 20 District Headquarters Hospitals and 64 Operation Theatres in 64 Taluk Head Quarters Hospitals have been provided with Air-Conditioning facilities. In the financial year 2003-2004, 25 Operation Theatres in 25 Taluk Hospitals were provided with Air-Conditioning facilities.
As per the policy decision taken by the Government, generators were provided to the District Headquarters Hospitals, Taluk Headquarters Hospitals and Non Taluk Hospitals in a phased manner.?

Again as per the policy decision taken by Government, 75 KVA Generators were sanctioned for 6 District Headquarters Hospitals, and in the year 2003-2004, 3 District Headquarters Hospitals were sanctioned with 75 KVA Generators.

In order to provide better communication facilities to Government Hospitals, Telephone facilities are being provided in a phased manner. In the year 2001-2002, 21 Joint Directors of Medical and Rural Health Services and Family Welfare were provided with Telephone facilities.? Again in the Year 2003-2004, 27 Taluk Headquarters Hospitals were provided with Telephone facilities.
With a view to impart proper Nursing Care to Rural Public and provide opportunity to students from rural area to take nursing profession as a career, seven new schools of Nursing have been started in the following District Headquarters Hospitals and 50 students are trained in each of these institutions:

To curtail the highly adverse sex ratio and to prevent female foeticide, the Pre-conception and Pre-Natal diagnostic Techniques (Regulation and Prevention of Misuse) Act 1994 is being implemented effectively in this state. State Level, District level, and sub district level advisory committees have been formed. According to the Act, 2534 Scan centers have so far been registered and constant vigil is being maintained to prevent sex detection. So far 90 unregistered scan centers have been identified and cases have been filed against 61 scan centers. Judgement has been delivered in 43 cases.

??????????? There are about 10 lakh persons infected by TB in Tamil Nadu. About 1.5 lakh people contact TB disease every year. Among them, about 60 thousand are infectious. There are 10 TB Sanatoria in Tamil Nadu-6 under the Control of Director of Medical Education, two under the control of Director of Medical and Rural Health Services and 2 private.

??????????? In all, there are 3654 beds in the State for treating acutely-ill TB patients. All the TB patients are getting treatment under the Directly Observed Treatment Short Course in the nearest Treatment Centres. There are 644 Microscopy Centres for sputum examination for diagnosis. There is one State TB Officer in the rank of Additional Director to coordinate and supervise the Programme in all the three Directorates. At the District level there is a District TB Control Society headed by the District Collector. The District TB Officer is coordinating the Programme at the District level. The Programme is getting technical assistance from WHO and TB Research Centre (ICMR) Chennai. Central Assistance under the external assistance component is provided to meet the entire expenditure of Drugs, Logistics and the establishment expenditure of Contractual staff including IEC activities. The State Government meets the entire expenditure towards staff of 30 District TB Centres. In the year 2003-2004, 6.4 lakhs patients were examined, about 90 thousands patients were diagnosed and treated. This year, it is proposed to involve many more NGOs, Private Nursing Homes and Private Doctors, so that we will diagnose more patients. This will enable to reduce incidence of TB to a greater extent.
